ENDOMETRIOSIS – What is it?

Endometriosis is an extremely common but long-term condition in which tissue similar to the tissue that forms the lining of the uterus grows outside of the uterine cavity. The lining of the uterus is called the endometrium. Endometriosis occurs when endometrial like tissue grows on your ovaries, bowel and tissues lining your pelvis. It is unusual for the tissue to spread to other areas beyond the pelvic region, however this is not impossible.

The hormonal changes of the menstrual cycle affect the misplaced endometrial tissue causing the area to become inflamed and painful. This means the tissue will grow, thicken and break down. Over time, the tissue that has broken down has nowhere to go and becomes trapped in the pelvis, causing inflammation, pain and the formation of scar tissue. Endometriosis is the second most common gynaecological condition in the UK, affecting 1.5 million women, but getting a diagnosis can take several years.

ENDOMETRIOSIS SYMPTOMS

The symptoms of Endometriosis can vary with some women experiencing mild symptoms, but others suffering moderate to severe. Because of this and the fact that endometriosis can sometimes be difficult to diagnose, currently the average length of time in the UK from an initial consultation to a full diagnosis is over 7 years.

The main symptoms of Endometriosis are:

Pain in your lower tummy or back (pelvic pain) – usually worse during your period.
Pain during or after sex.
Pain when using the toilet during your period.
Feeling sick, constipation, feeling bloated.
Fertility problems, difficulty getting pregnant.
Fatigue.
Headaches.

Endometriosis is classified into 4 stages depending on the location within the body and the amount of tissue present.

STAGE 1 endometriosis is also called minimal. In these cases there a few small implants of endometrium like cells (tissue). They are often found on organs or within the pelvis or abdomen.

STAGE 2 or mild endometriosis means there are more implants than in stage 1. There may also be some scar tissue present as the cells embed deeper in to the tissue.

STAGE 3 Also known as moderate endometriosis. This stage means there are many deep visible implants. It’s also likely that there are cysts present on the ovaries as well as thick scar tissue these are called adhesions.

STAGE 4 Severe endometriosis or stage 4 is the most widespread stage of the condition. Here, it is likely that there will be lots of deep implanted cells around the body, also having large cysts on either one or both of the ovaries.

Nobody knows what causes Endometriosis, why some suffer and others don’t, but the symptoms are known to be debilitating and according to Endometriosis Uk the condition costs the UK economy £8.2bn a year in treatment through loss of work and healthcare costs.

Currently there is no cure, and treatment is limited to different strength painkillers, hormonal contraception, or surgery to remove/cut away the scar tissue. In extremely severe cases, the only option may be a full hysterectomy to remove part or all of the affected organs.
